2) A resume or CV. (As we cannot guarantee anonymity, you may want to remove references to your real name)
3) Examples of prior writings or contributions in other venues, including but not limited to other web fora.
If your application is reviewed favorably you will receive further instructions via email. Turn around time is usually less than one week.
